Botox vs Dysport vs Xeomin: How These Wrinkle Treatments Compare

Walk into a consultation about wrinkles and you may hear three names instead of one. Botox, Dysport, and Xeomin all soften lines, and all three are FDA approved. So why does your provider suggest one over another?

They are cousins, not clones. Here is what actually separates them.

What They All Have in Common

Start with the shared ground, because it is most of the story.

All three are made from botulinum toxin type A. All three work the same way. When injected into a specific muscle, they temporarily block the nerve signal telling that muscle to contract. The muscle relaxes, and the skin above it smooths out.

This is why they only work on one kind of wrinkle. Lines caused by movement, like frown lines between the brows or crow's feet when you smile, respond well. Lines caused by sun damage, lost volume, or sagging do not. Those need different treatments entirely.

The Real Difference: What Is in the Vial

Here is where they split apart.

Botox and Dysport both wrap the active toxin in extra proteins, sometimes called accessory or complexing proteins. These help stabilize the product in the vial.

Xeomin does not. It is filtered during manufacturing to strip those extra proteins away, leaving just the active molecule. Providers sometimes call it the naked or pure toxin.

That single difference drives most of the others.

Three things on that table deserve a closer look.

Spread Is a Tool, Not a Flaw

Dysport tends to travel a little further from where it is injected. Xeomin stays the most contained, with Botox in between.

Neither is better in general. Wider spread can be useful across a broad area like the forehead, where even coverage matters. Tighter spread is better when precision counts, such as near the brow or eye, where you do not want the product drifting into muscles that hold your eyelid up. A skilled injector picks based on the area, not on a favorite brand.

Units Do Not Transfer

This one confuses people constantly. Twenty units of one product is not twenty units of another. The scales were set independently. Roughly speaking, one unit of Botox is comparable to around two and a half to three units of Dysport, while Xeomin runs close to one to one with Botox.

So if you compare prices between clinics based on cost per unit alone, you can easily mislead yourself. Ask what product is used and how many units your treatment needs.

Duration Is Mostly About You

Every product lands in the same general range of about three to four months. No brand has been clearly shown to outlast the others in well-designed comparisons.

What actually moves the needle is you: your metabolism, how strong the treated muscle is, how much product was used, and which area was treated. Two people can get the same treatment and see it fade on different schedules.

When One Might Suit You Better

A few situations where the choice genuinely matters.

If your treatment seems to have stopped working. Some long-term users find their results fade faster over the years. One suspected cause is the immune system building antibodies against those extra proteins. Since Xeomin has none, some providers suggest it for people in this situation. This is not common, but it is real.

If you have a milk protein allergy. Dysport contains a milk protein, so it is generally avoided for people with that allergy. Worth mentioning during your health history.

If you want the fastest visible change. Dysport often shows up a couple of days sooner. If you have an event coming up, timing may matter. That said, some providers note there is no firm clinical proof that one reliably beats the others on speed, and individual response varies.

If precision is the priority. For small, targeted areas, the more contained spread of Xeomin or Botox may be the better fit.

What Matters More Than the Brand

Here is the part worth remembering. For most people, all three products work well, and the choice between them is a smaller factor than almost anything else.

Injector skill matters more. Where the product goes, how deep, how much, and how the dose is balanced across muscles will shape your result far more than the label on the vial. The same product in two different hands can look very different.

Dose matters too. Softer, natural results usually come from thoughtful dosing rather than a specific brand. That idea sits behind the trend toward smaller preventative doses that more people are asking about.
